INDEPENDENT CONTRACTOR AGREEMENT

This Agreement is made and entered into on                               by and between:

Blush Beauty, a Michigan LLC company, located at 1111 Godfrey Ave, Grand Rapids, MI,
and                                        an independent contractor located at [Artist's Address] ("Contractor").

​

Services Provided
Contractor agrees to provide professional hair and/or makeup services ("Services") as requested by the Company, including but not limited to:

• Bridal hair and/or makeup

• Event or editorial styling

• On-site or studio services

Services will be provided on a project-by-project basis, scheduled by Blush Beauty.

Independent Contractor Relationship
Contractor is not an employee of Blush Beauty.
Contractor shall provide Services using their own tools and materials unless otherwise arranged.
Contractor has the right to control the manner and means of performing the Services.

​

Compensation
Contractor will be paid per job upon completion of each assignment.
Payment will be made within 24 hours of service date.
Contractor is responsible for their own taxes, insurance, and business expenses.

Scheduling & Availability
Blush Beauty will offer job opportunities, which the Contractor may accept or decline.
Once an assignment is accepted, the Contractor agrees to show up on time and complete services as scheduled.

Professional Standards
Contractor agrees to:

• Arrive on time and fully prepared

• Present themselves in a clean, professional manner

• Provide high-quality services and maintain sanitary tools and workspaces

• Communicate respectfully with clients, team members, and Blush Beauty representatives

 

Cancellations & No-Shows
Contractor must notify the Company at least 48 hours in advance if unable to attend a scheduled job.
Unexcused no-shows or last-minute cancellations may result in termination of this Agreement.

Confidentiality

Contractor agrees not to disclose or share confidential business information, client lists, or pricing details without written permission from Blush Beauty.

Tax Documentation
Contractor agrees to provide a completed W-9 form to Blush Beauty upon request and no later than the end of the calendar year. This form will be provided by Blush Beauty. Failure to provide the W-9 may result in a delay in payment or suspension of future job opportunities.

Term & Termination
This Agreement begins on the date signed and remains in effect until terminated by either party.
Either party may terminate with written or verbal notice at any time.

Liability & Insurance
Contractor agrees to carry their own liability insurance and is responsible for any claims arising from their own negligence.
Blush Beauty is not liable for injury or damage caused by the Contractor’s actions.

Amendments
The Parties agree that any amendments made to this Agreement must be in writing and signed by both Parties.
As such, any amendments made by the Parties will be applied to this Agreement.

This Agreement constitutes the complete and entire understanding between the Parties regarding its subject matter and supersedes all prior agreements, understandings, inducements, and conditions, whether express or implied, oral or written. The explicit terms of this Agreement override any previous performance or trade practices that conflict with its terms.
